The Urban Health Initiative (UHI) facilitates UChicago Medicine’s efforts to address the complex health and social needs of residents on the South Side of Chicago. UHI uses a collaborative, community-based partnership approach to create access to high-quality and culturally competent healthcare information and services, empowering community members to live their healthiest lives. UHI’s hospital-based Violence Recovery Program successfully reduces re-injury risk and promotes comprehensive recovery. Liaisons in Care (LinC) community health workers provide care coordination, at-home wellness checks, and community outreach to improve healthcare access and autonomy for residents in our service area.
